Texas Consumer Information & Patient Rights

In accordance with Texas law (House Bill 4224 and Section 181.105 of the Texas Health and Safety Code), the following information is provided to help consumers understand their rights and available resources.

At Hasta La Raiz Neurodiversity Affirming Psychotherapy & Support, we believe in transparency, respect, and supporting our clients every step of the way. The information below is provided to help you understand your rights and available resources under Texas law.

Requesting Your Health Care Records

You have the right to request access to your health care records from The Space Between Therapy.

To request your records:

  • Please submit a written request to: cecilia@hastalaraizneurodiversityaffirming.com

  • Include the client’s full name, date of birth, and a brief description of the records you are requesting.

  • A signed authorization form may be required before records can be released.

  • Requests are processed in accordance with applicable state and federal laws and within required timeframes.

The Texas Behavioral Health Executive Council oversees the licensing of behavioral health professionals in Texas.

Filing a Consumer Complaint

Clients may file a complaint with the Texas Office of the Attorney General’s Consumer Protection Division if they have concerns related to services provided.
